New patients do not need a referral to make an appointment. If you must cancel an appointment, please give us 24 hours notice.
New patients
If you are coming to Wooster OB-GYN for the first time, please complete and bring with you a New patient registration form, your insurance card and a list of the medications you currently take and their dosages.
We can request your prior health records from your other doctors, but if you have them at home, please bring them to your appointment. Please read and sign our Financial policy and bring it with you.
After-hours
An answering service takes our calls after hours and forwards them to the appropriate staff member. You may find it helpful to review our Frequently Asked Questions (FAQs) before you call.
